Our Legal Duties
We are required by law to maintain the privacy of your protected health information (PHI) and to provide you with this Notice. We must abide by the terms of this Notice currently in effect.

Uses and Disclosures
We may use or disclose your PHI for purposes of treatment, payment, and healthcare operations. This may include sharing information with other healthcare providers involved in your care, billing insurance companies, and conducting internal quality assurance and administrative functions.

Special Protections for Certain Health Information
Some types of health information receive additional protections under federal and state law. This includes:

Substance Use Disorder (SUD) treatment records, which may be protected under 42 C.F.R. Part 2 when received from federally assisted programs.
Reproductive health information, which is subject to additional federal privacy protections.
Premier Wellness Centers will comply with all applicable federal and state laws regarding the use and disclosure of this information. Where required, we will obtain appropriate authorization before disclosing such protected information, except as permitted or required by law.

 

Your Rights

You have the right to:

• Access and obtain a copy of your medical records
• Request corrections to your health information
• Receive a list of certain disclosures of your information
• Request restrictions on how your information is used or shared
• Request confidential communications
• Receive a copy of this Notice

You may revoke your authorization at any time, except to the extent we have already acted on it.

Changes to This Notice
We reserve the right to change this Notice and our privacy practices at any time. Any revised Notice will be posted at our facility and made available on our website.

Complaints
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services. You will not be penalized or retaliated against for filing a complaint.
